Position Details

Title: Certified Alcohol and Drug Counselor

Location: 12360 E Burnside St. Portland, OR 97233

Schedule: In-person, Monday to Friday, 8:30AM - 5:00PM

Wage Range: $24.85 - $33.65, non-exempt

Position Overview

The Spirit of Healing CADC Care Coordinator collaborates with Native American or Alaskan Native members facing complex health and psychosocial issues, providing care coordination and ASAM assessments. This role supports members in accessing services and acts as a bridge between the community and health/social systems.

Responsibilities
  • Administer ASAM assessments and treatment plans, ensuring proper documentation.
  • Adhere to Trauma-Informed Care principles and educate providers accordingly.
  • Schedule discharges from hospitals or inpatient settings.
  • Coordinate and update care plans focusing on self-management and member goals.
  • Utilize EMR systems for documentation and make independent care decisions.
  • Engage clients by addressing social, cultural, linguistic, and educational barriers.
  • Coordinate with community treatment providers.
  • Participate as part of a healthcare team, providing input and developing strategies.
  • Use Motivational Interviewing to respect client autonomy and foster engagement.
  • Facilitate communication between clients and providers.
  • Participate in outreach and care team meetings.
  • Respond to hospital outreach requests.
  • Collaborate on healthcare plans based on client needs.
  • Assist with transportation and referrals as needed.
  • Mentor and train new staff.
  • Identify and reduce barriers to care using a culturally sensitive approach.
  • Follow organizational policies and manage an effective caseload.
  • Attend scheduled work and participate in continuing education.
  • Complete reports timely and demonstrate teamwork.
