You’ll mentor staff, oversee care plans, and ensure personalized, compassionate resident care while maintaining accurate documentation.

Qualifications

• RN or LPN/LVN license current and in good standing.
• 3+ years of healthcare experience with supervisory exposure.

Responsibilities

• Oversee health and wellness operations on each shift in Assisted Living and Memory Care.
• Lead, coach, and inspire nursing staff, med techs, CMAs, and caregivers.
• Train team members through engaging, real-world learning.
• Maintain accurate charts and documentation.
• Assess, plan, and update individualized Clinical Care Plans.
• Identify health concerns early to help residents stay at home longer.
• Oversee medication management, training, and audits.
• Share on-call and Manager-on-Duty duties.
• Assist with onboarding new team members.

About The Role

We're searching for a dynamic, hands-on Clinical Care Coordinator to lead health and wellness operations in our Assisted Living and Memory Care units.

You'll be more than a nurse - you'll be a mentor, problem-solver, and advocate, ensuring every resident receives personalized, compassionate care while empowering your team to deliver their very best.

What You'll Do

• Ensure top-quality care on every shift (and step in to assist as needed).
• Lead, coach, and inspire nursing staff, medication techs, CMAs, and caregivers.
• Train team members with engaging, real-world learning.
• Maintain accurate charts and documentation.
• Assess, plan, and update individualized Clinical Care Plans.
• Spot and address health concerns early to help residents stay in their home longer.

What You Bring

• RN or LPN/LVN license (current & in good standing)
