Freedom Village Bradenton in Bradenton, FL is seeking a dynamic Clinical Care Coordinator to lead health and wellness operations in our Assisted Living and Memory Care units. You’ll mentor staff, coordinate personalized resident care, and ensure compassionate service that helps residents stay in their home longer.

RN or LPN license with 3+ years of healthcare supervisory experience, strong leadership and communication skills, and proficiency with documentation and computer systems are required.

Qualifications

• RN or LPN/LVN license current and in good standing.
• 3+ years of healthcare experience; supervisory experience a plus.
• Strong communication and leadership abilities.
• Proficiency with computer systems and documentation.
• Commitment to confidentiality and professionalism.
• Ability to pass background and drug screening.

Responsibilities

• Ensure top-quality care on every shift and assist as needed.
• Lead, coach, and inspire nursing staff, medication techs, CMAs, and caregivers.
• Train team members with engaging, real-world learning.
• Maintain accurate charts and documentation.
• Assess and update individualized Clinical Care Plans.
• Oversee medication management, training, and audits.
• Share on-call and Manager-on-Duty responsibilities.
• Assist with onboarding new team members.

ABOUT THE ROLE

We’re searching for a dynamic, hands-on Clinical Care Coordinator to lead health and wellness operations in our Assisted Living and Memory Care units.

You’ll be more than a nurse — you’ll be a mentor, problem-solver, and advocate, ensuring every resident receives personalized, compassionate care while empowering your team to deliver their very best.

WHAT YOU’LL DO
