Qualifications

• Bachelor's Degree in Social Services is preferred.
• Six years of work experience in Human Services is required.
• Experience with individuals coping with mental health conditions is required.
• Must have a Pennsylvania State Driver’s License with a clear driving history.
• Clear Criminal Background Check from PA State Police is necessary.
• Acceptable results from physical/drug screen/TB test are required.

Responsibilities

• Conduct medical appointments to follow up on treatments ordered by medical professionals.
• Monitor medical records and ensure all treatments are documented according to policy.
• Ensure timely ordering of medications per physician's orders.
• Accompany individuals to medical appointments and advocate for their needs.
• Maintain organized medical and medication records in compliance with regulations.
