Clinical Care Coordinator (LPN) – Hillcrest Country Estates

Location: Papillion, NE
Service Line: Hillcrest Country Estates
Schedule: Full-Time

Hillcrest Health & Living is looking for a confident, clinically strong, and highly organized Clinical Care Coordinator (LPN) to join the nursing leadership team at Hillcrest Country Estates in Papillion, NE.

This is a great opportunity for an experienced nurse who enjoys combining hands-on clinical leadership, care coordination, problem-solving, and team development. As a Clinical Care Coordinator, you will help guide the care of post-acute patients from admission through discharge while ensuring strong clinical outcomes, regulatory compliance, and a positive patient experience.

At Hillcrest, our mission is simple: Inspiring people to live their best lives. In this role, you will help bring that mission to life by leading quality clinical care, supporting the nursing team, and helping patients safely move toward their next level of independence.

What You’ll Do

As a Clinical Care Coordinator, you will provide clinical leadership and oversight while helping coordinate each patient’s care throughout their post-acute stay.

You will:

  • Coordinate clinical care from admission through the discharge process.
  • Help oversee daily and ongoing nursing staffing needs.
  • Complete daily rounds on patients and identify needs requiring follow-up.
  • Monitor the neighborhood for quality, safety, and regulatory compliance.
  • Ensure call lights, infection-control requirements, and other clinical processes are completed appropriately.
  • Review fall risks, incidents, and prevention strategies and ensure care plans are updated as needed.
  • Monitor skin integrity processes, including weekly wound rounds and documentation.
  • Review weights and other clinical information for accuracy and appropriate follow-up.
  • Ensure Nurse Tech care plans and patient care plans remain current as needs change.
  • Participate in daily multidisciplinary meetings and communicate clinical concerns requiring team follow-up.
  • Help ensure clinical tools and care pathways are being used effectively to manage changes in patient condition.
  • Prepare the neighborhood and care team for new admissions.
  • Ensure appropriate nurse-to-nurse communication occurs before a patient arrives.
  • Help create a smooth and welcoming admission experience for patients and families.
  • Review electronic medication records for missed medications, unresolved concerns, and medication-pass compliance.
  • Provide coaching, education, and clinical guidance to nursing team members.
  • Partner with physicians, therapy, social services, nursing, and other interdisciplinary team members.
  • Step in to provide direct nursing care when necessary to support safe staffing and quality care.
  • Participate in leadership meetings and clinical improvement initiatives.
What We’re Looking For

We are looking for an LPN who can see both thebig picture and the clinical details.

The right candidate will be someone who can walk onto a neighborhood, quickly understand what is happening, identify areas needing attention, and work with the team to get things moving in the right direction.

This role is ideal for a nurse who enjoys leadership but still wants to stay connected to patient care.

Ideal candidates will have:

  • Strong clinical judgment and critical-thinking skills.
  • Confidence leading and supporting nursing team members.
  • Strong knowledge of post-acute and skilled nursing care.
  • Ability to recognize changes in patient condition and determine appropriate next steps.
  • Strong assessment and care-planning skills.
  • Experience monitoring quality measures and regulatory requirements.
  • Strong communication and interdisciplinary collaboration skills.
  • Ability to coach, mentor, and hold team members accountable.
  • Excellent organization and follow-through.
  • Strong attention to documentation and clinical accuracy.
  • Ability to prioritize multiple needs in a fast-paced clinical environment.
  • A proactive approach to identifying concerns and implementing solutions.
  • Ability to remain calm, flexible, and professional when situations change quickly.
Education & Credentials
  • Current Nebraska LPN license required.
  • Bachelor’s degree in Nursing preferred.
  • At least two years of experience in skilled nursing, acute care, or post-acute care required.
  • At least two years of supervisory experience in skilled nursing, acute care, or post-acute care preferred.
  • American Heart Association Basic Life Support (BLS) certification required.
