Qualifications

  • LPN license in Nebraska or a compact state is required.
  • Long-term care or skilled nursing facility experience is required.
  • Previous charge nurse, unit manager, or clinical care coordination experience preferred.
  • Strong understanding of post-acute care and Medicare documentation.
  • Ability to support admissions, transitions, and discharge planning.
  • Strong accountability, communication, and documentation skills.
  • One weekend day schedule and rotating on-call every fourth weekend.

Responsibilities

  • Provide clinical oversight and support for assigned neighborhoods, residents, patients, and team members.
  • Participate in staffing oversight and daily rounds to meet patient needs.
  • Monitor care delivery, call lights, infection control, fall prevention, and care plan updates.
  • Attend daily multidisciplinary stand-up meetings and follow up items.
  • Ensure INTERACT tools and care paths are implemented and updated.
  • Support team education on Medicare documentation and required stays.
  • Prepare rooms and equipment for new admissions; ensure handoffs to the next shift.
  • Serve as backup for Admissions Coordinator during vacations or absences.
  • Facilitate patient-centered transitions from admission to discharge.
  • Partner with nurse managers on performance reviews and coaching.
  • Review EMAR records for missed meds and proper administration.
  • Participate in QAPI meetings and assist with dashboards and improvement plans.
  • Fill in on the floor as a nurse to support staffing as needed.
  • Work varied shifts and on-call every fourth weekend.

Hillcrest Health & Living is looking for an experienced, organized, and clinically strong Clinical Care Coordinator to join our team at Hillcrest Firethorn in Lincoln, NE.

As the Clinical Care Coordinator, you will play an important leadership role in supporting clinical operations, guiding team members, coordinating care, and helping ensure strong outcomes from admission through discharge.

What You’ll Do
  • Provide clinical oversight and support for assigned neighborhoods, residents, patients, and team members.
  • Participate in the oversight of daily and ongoing staffing needs.
  • Complete daily rounds to ensure patient, resident, and team member needs are being met.
  • Monitor care delivery, call light response, infection-control practices, fall prevention, care plan updates, wound rounds, weights, documentation, and dining room supervision.
  • Attend daily multidisciplinary stand‑up meetings and bring forward items needing follow‑up.
  • Ensure INTERACT tools, care paths, and other clinical resources are implemented, reviewed, and updated as needed.
  • Support team member education related to Medicare documentation, ADL documentation, coverage guidelines, and required documentation for Medicare stays.
  • Help prepare for new admissions by ensuring rooms are ready, equipment is available, nurse‑to‑nurse report has been received, and team members are prepared to welcome the patient.
  • Serve as backup support for the Admissions Coordinator during vacations, absences, or times out of the office.
  • Support patient‑centered transitions from admission through discharge.
  • Partner with nurse managers to complete Nurse Tech performance reviews, coaching, and counseling.
  • Review electronic medication administration records for missed medications, unresolved flags, timeliness of med pass, and correct med pass procedures.
  • Participate in QAPI meetings and assist with dashboards, performance improvement plans, and follow‑up actions.
  • Fill in on the floor as a nurse when needed to support adequate staffing and patient care.
  • Work occasional varied shifts or hours as needed to observe care, complete evaluations, support coaching, and meet operational needs.
  • Participate in rotating on‑call responsibilities every fourth weekend.
  • Communicate clearly and professionally with patients, residents, families, providers, nurses, Nurse Techs, therapy, admissions, and leadership.

The right candidate will be comfortable functioning as a clinical leader on the floor, supporting team members, managing changing priorities, and keeping patient and resident care at the center of each decision. This role is a strong fit for someone with unit manager, charge nurse, or similar leadership experience who is confident stepping in, solving problems, and helping the team stay focused on quality care.

Ideal Candidates Will Have
  • Active LPN license in Nebraska or a compact state required.
  • Long‑term care or skilled nursing facility experience required.
  • Previous charge nurse, unit manager, clinical coordinator, or similar leadership experience strongly preferred.

  • Confidence coaching team members, addressing concerns, and supporting performance expectations professionally.
  • Strong attention to detail with care plans, EMAR review, documentation, wounds, weights, fall prevention, and regulatory expectations.
  • Ability to stay calm, flexible, and professional in a fast‑paced care environment.
  • A team‑first approach with the professionalism to communicate directly, respectfully, and solution‑focused.
  • Willingness to work a schedule that includes one weekend day and rotating on‑call every fourth weekend.
Education & Credentials
  • Active Licensed Practical Nurse license in Nebraska or a compact state required.
  • Long‑term care or skilled nursing facility experience required.
  • Previous charge nurse, unit manager, clinical care coordination, or nurse leadership experience strongly preferred.
  • Basic Life Support certification through the American Heart Association required.
