The Coordinating Center in Maryland is seeking a Clinical Care Coordinator for a full-time teleworking role with regular client visits in Prince George's County and surrounding areas. You will partner with families to develop individualized care plans and coordinate medical and community services.
You will maintain documentation across electronic databases, work toward CCM certification within two years, and may receive a $1500 stipend after certification.
This is a full-time teleworking position, with the expectation of visiting clients regularly in Prince George's County area + provider visits in surrounding counties.

As a Clinical Care Coordinator, you will provide day-to-day care management and coordination services within the Model Waiver Program. Partnering with families and caregivers, you will work directly with clients on creating an individualized plan of care with goals to help them remain independent in the community setting of their choice. This position is a hybrid of teleworking and client visits in the community setting including client homes, facility settings, school/day programs, and hospitals.
The salary for this position is $70,000 and comes with all of our full time benefits. This position requires the CCM certification within two years of employment, with a $1500 stipend upon completion. Candidates who already have their CCM would have a salary of $72,000.
