Job summary

Focus Home Health is seeking a Clinical Care Coordination Specialist (LVN) to join our intake team and ensure referrals are clinically appropriate, complete, and ready for admission. This remote California role supports referral partners and patients, acting as the primary clinical resource for admission readiness.

In this position you will review referrals, collaborate with physicians and internal teams, assist with insurance authorizations, and document thoroughly in our EMR.

Qualifications

• Current California LVN license in good standing.
• Minimum of two (2) years LVN experience required.
• Home health experience strongly preferred.
• Experience in Home Health Intake, Patient Access, Care Coordination, Admissions, or related healthcare role preferred.
• Knowledge of Medicare and insurance verification processes preferred.

Responsibilities

• Review referrals for clinical appropriateness and home health eligibility.
• Evaluate physician orders and supporting documentation.
• Collaborate with physicians and referral sources to obtain required records.
• Assist with insurance authorization and payer-specific admission requirements.
• Serve as a clinical resource for Intake Coordinators and leadership.
• Communicate referral status and admission readiness to partners.
• Maintain accurate documentation in agency systems.

As the clinical partner to our intake team, the Clinical Care Coordination Specialist (LVN) helps bridge the gap between referral receipt and patient admission, ensuring every patient is clinically appropriate, compliant, and ready to begin services. This position works closely with the Intake Team and serves as the primary clinical resource supporting referral review, admission readiness, and patient access to care.

What You'll Do

• Review incoming referrals for clinical appropriateness and home health eligibility
• Evaluate physician orders, hospital documentation, diagnoses, and supporting records
• Collaborate with physicians, referral sources, and internal teams to obtain required documentation
• Assist with insurance authorization and payer-specific admission requirements
• Serve as a clinical resource for Intake Coordinators and agency leadership
• Communicate referral status and admission readiness to referral partners
• Support timely patient admissions and positive referral conversion outcomes
• Maintain accurate documentation within agency systems
• Ensure compliance with Medicare, Medi-Cal, Managed Care, and agency requirements

What We're Looking For

• Current California LVN license in good standing
• Minimum of two (2) years of LVN experience required
• Home health experience strongly preferred
• Previous experience in Home Health Intake, Patient Access, Care Coordination, Admissions, Case Management, or a related healthcare role preferred
• Experience with Homecare Homebase (HCHB) or similar electronic medical record systems preferred
• Knowledge of Medicare, insurance verification, authorization processes, and referral management preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and customer service abilities
• Strong organizational skills and attention to detail
• Ability to thrive in a fast-paced, team-oriented environment
