Focus Home Health in San Jose, CA is seeking a Clinical Care Coordination Specialist (LVN) to join our on-site team. You will serve as the clinical expert within the intake process, ensuring referrals are clinically appropriate, complete, and ready for admission while supporting patients and referral partners.

As the clinical partner to the intake team, you will bridge referral receipt and patient admission, ensuring compliance and readiness to begin services.

Qualifications

• Current California LVN license in good standing.
• Minimum of two years LVN experience required.
• Home health intake/admission experience preferred.
• Experience with Medicare, insurance verification, authorization processes, and referral management preferred.
• Strong communication and organizational skills are essential.

Responsibilities

• Review incoming referrals for clinical appropriateness and home health eligibility.
• Evaluate physician orders, hospital documentation, diagnoses, and supporting records.
• Collaborate with physicians, referral sources, and internal teams to obtain required documentation.
• Assist with insurance authorization and payer-specific admission requirements.
• Serve as a clinical resource for Intake Coordinators and agency leadership.
• Communicate referral status and admission readiness to referral partners.
• Support timely patient admissions and positive referral conversion outcomes.
• Maintain accurate documentation within agency systems.
• Ensure compliance with Medicare, Medi-Cal, Managed Care, and agency requirements.

What We're Looking For

• Current California LVN license in good standing
• Minimum of two (2) years of LVN experience required
• Home health experience strongly preferred
• Previous experience in Home Health Intake, Patient Access, Care Coordination, Admissions, Case Management, or a related healthcare role preferred
• Experience with Homecare Homebase (HCHB) or similar electronic medical record systems preferred
• Knowledge of Medicare, insurance verification, authorization processes, and referral management preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and customer service abilities
• Strong organizational skills and attention to detail
• Ability to thrive in a fast-paced, team-oriented environment
