Job Summary

Assist in all aspects of medical practice under the supervision of a provider, including patient care coordination, executing administrative and clinical procedures, and participating in the interdisciplinary team.


Essential Job Functions

  • Assist healthcare providers during home visits as needed.
  • Coordinate durable medical equipment, supplies, and home health services.
  • Provide administrative support by scheduling appointments and tracking referrals.
  • Coordinate access to prescribed medications.
  • Maintain a safe environment.
  • Assist with examinations and patient care as required.
  • Check and report on equipment functionality and safety.
  • Follow infection control and OSHA regulations.
  • Perform venipuncture, specimen collection, and labeling.
  • Assist with procedures like EKG, vaccinations, and medication administration.
  • Review medical records and follow up on missing results.
  • Conduct history and needs assessments with patients.
  • Identify patients with potential health needs and coordinate care.
  • Manage non-clinical needs of patients with chronic conditions.
  • Participate in team huddles and care management activities.
  • Follow up with patients/families on non-clinical matters.
  • Serve as a resource and advocate for patients and families.
  • Educate patients on appropriate use of medical services and improve health literacy.
  • Demonstrate cultural sensitivity and knowledge of data collection tools.
  • Maintain compliance with Medicare/Medicaid, federal, and state regulations.
  • Adhere to HIPAA requirements.
  • Engage in professional development activities.
  • Perform duties independently and meet workload expectations.
  • Report concerns promptly to leadership.
  • Exhibit excellent communication and problem-solving skills.
  • Participate in multidisciplinary palliative care team meetings.
Qualifications

Required Education: High School Graduate or equivalent


Required Experience: 1 year in a clinical setting, hospital, or physician office experience


Required Licenses and Certifications: Medical Assistant Certificate, Phlebotomist, CPR


Required Knowledge, Skills, and Abilities: Strong computer and data entry skills, customer service skills, bilingual in English and Spanish
