Job Description :

GENERAL OVERVIEW:

This role collaborates with palliative care providers and other members of the interdisciplinary team to support effective outcomes and shared goals for patients and their families. The professional role model utilizes expertise in care management to promote a collaborative environment that supports excellence in care and optimal resource utilization.


ESSENTIAL RESPONSIBILITIES

  1. Assess patients' physical, psychosocial, and economic needs to facilitate effective care transition planning across various care levels.
  2. Contribute to developing the transition plan for patients and families.
  3. Document, verify, and validate data required to monitor interventions and outcomes, including patient interviews and chart reviews related to readmission.
  4. Ensure compliance with accreditation and regulatory standards; incorporate performance improvement and customer service principles into all responsibilities.
  5. Participate in Goals of Care discussions with patients or their proxies.
  6. Coordinate care among Highmark entities such as Endorsed LLC, Healthcare@Home, and Journeys.
  7. Engage in data tracking initiatives to improve clinical outcomes.
  8. Participate in quality improvement initiatives and ensure appropriate use of order sets.
  9. Use the nursing process to assess, plan, evaluate, and implement patient care tailored to individual needs, in accordance with physician directives and hospital policies.
  10. Perform symptom assessment and management, including medication titration.
  11. Maintain ongoing education and professional development as required by AHN.
  12. Perform additional duties as assigned.

QUALIFICATIONS:

Minimum

  • Bachelor's Degree in Nursing or relevant experience/education as determined by the company in lieu of a degree.
  • Act 34 Criminal Background Clearance
  • Act 33 Child Abuse Clearance
  • Act 73 FBI Fingerprinting Criminal Background Clearance
  • Current PA RN licensure or multi-state licensure through eNLC.
  • CPR certification.

Preferred

  • Knowledge of discharge planning and community resources
  • One year experience in Home Care or hospice
  • Case Management experience
  • One year experience in Critical Care
  • Strong time management and communication skills
