REM Community Services, a part of Sevita, seeks a Clinical Specialist to deliver services in residential, vocational, or in-home settings. You'll support individuals with developmental and related challenges, ensuring plans meet regulatory standards and are person-centered.

You will conduct assessments, document progress, and collaborate with IDT while promoting dignity and rights. A BS in a human services field and 2+ years of related experience are preferred; local travel may be required.

Competenze

• Bachelor’s Degree in a human services field is required.
• Two years of related experience preferred.
• Training in behavior modification techniques or experience with behavior management.
• Knowledge of populations with developmental disabilities, brain injury, or mental health needs.
• Reliable, responsible, and committed to the mission.

Mansioni

• Ensure services adhere to Individual Plans, quality standards, and regulations.
• Conduct structured assessment interviews with parents/care providers.
• Observe individuals in home, school, or community settings.
• Review social history, complete functional assessments, and baseline behavior data.
• Prepare and submit assessment summaries and behavior support plan components to IDT.
• Advocate for rights of individuals served and present behavior plans at reviews.
• Document progress, maintain records, and ensure confidentiality.
• Maintain professional relationships with individuals, families, and case managers.
• Attend meetings and train staff on Individual Support Plans and behavior plans.
• Assist with daily living activities and accompany to medical appointments as needed.
