REM Community Services, part of Sevita, is seeking a Clinical Specialist to provide services in residential, vocational, or in-home settings, delivering invaluable support to the individuals you serve. You will ensure plans meet regulatory requirements, perform assessments, observe daily living activities, and collaborate with an interdisciplinary team.

A Bachelor’s in human services and two years of related experience are required, with a valid driver’s license and willingness to travel

Competenze

• Bachelor’s Degree in a human services field.
• Two years of related experience.
• Training in behavior modification techniques or experience providing behavior management.
• Knowledge of developmental disability, brain injury, or mental health populations.
• Valid driver’s license and auto insurance if transporting individuals.
• All state-required trainings completed in mandated timeframes.

Mansioni

• Ensure support plans adhere to Individual Plans, quality standards, and regulatory requirements.
• Complete structured assessment interviews with parents/care providers.
• Conduct observation of individuals in home, school, or community.
• Review social history, complete functional assessments, and set behavior baselines.
• Write and submit assessment summaries and behavior plan components to the IDT.
• Advocate for rights and present behavior plan information to review committees.
• Document progress; maintain confidentiality; complete billing documentation.
• Maintain professional relationships with individuals, families, guardians, and case managers.
• Support and train staff in implementing Individual Support Plans and conduct orientations.
• Assist with activities of daily living and accompany to medical appointments as assigned.
