Job summary

Mentor Community Services, part of the Sevita family, seeks a Clinical Specialist to deliver services in residential, vocational, or in-home settings. You will assess, develop, and implement behavior support plans while advocating for client rights and coordinating with interdisciplinary teams.

You will document progress, train staff, and ensure compliance with safety and regulatory standards, delivering compassionate, person-centered care.

Qualifications

• Bachelor’s degree in a human services field.
• Two years of related experience.
• Training in behavior modification techniques and/or behavior management experience.
• Knowledge of populations such as developmental disabilities, brain injury, or mental health.

Responsibilities

• Ensure support plans adhere to plans, standards, and regulations.
• Complete structured assessment interviews with parents/care providers.
• Conduct observations in home, school, or community settings.
• Review social history, complete functional assessments, and target baselines.
• Submit assessment summaries and behavior plan components to IDT or leadership.
• Advocate for client rights and present behavior plan information to review committees.
• Document progress, maintain confidentiality, and handle billing documentation.
• Maintain professional relationships with individuals, families, guardians, and case managers.
• Report abuse or neglect and medical/behavioral incidents per policy.
• Support staff in implementing plans and conduct trainings as assigned.
• Assist with daily living activities and accompany to medical appointments as needed.
