Job summary

EGA Associates, LLC is seeking a Licensed Practical Nurse/ Licensed Vocational Nurse to serve as a Clinical Associate at a Primary Care Community Based Outpatient Clinic for Veterans in Pottsville, PA. You will support patient-centered care, care coordination, and clinic workflow as part of the Patient Aligned Care Team.

Responsibilities include triage and care delivery, collaboration with PACT staff, and telehealth readiness. Strong teamwork and EHR skills are essential; travel is negligible.

Qualifications

• Current, full, active LPN license in the state where the clinic is located.
• Current Basic Life Support certification or ability to obtain certification before patient care.
• Ability to complete VA and EGA credentialing, background screening, training, and onboarding requirements.
• Knowledge of basic patient care, medical terminology, aseptic technique, infection control, confidentiality, and clinical documentation.
• Ability to work effectively in an interdisciplinary, outpatient environment.

Responsibilities

• Provide evaluation and care to patients within LPN scope of practice.
• Collaborate with PACT to develop care plans for patient panels.
• Manage clinic workflow and room assignments to ensure timely care.
• Provide patient guidance as they move through the clinic.
• Perform basic patient-care services and support skills.
• Assist providers with patient care and documentation.
