Job summary

ncgCARE is seeking a Clinical Assessment Specialist in Belle Haven, VA to perform CNAs, complete intake paperwork, and populate patient profiles in the EHR. You will collaborate with guardians, providers, and MCOs to coordinate services and referrals, ensuring timely authorizations.

Responsibilities include communicating with care teams, maintaining documentation, and supporting leadership with accurate assessments and follow-ups. A Master’s degree and licensure eligibility are required.

Qualifications

• Resident in Counseling or a full Mental Health license (e.g., LPC, LCSW, LMFT).
• Strong organizational and time management skills.
• Proficiency in G-Suite and EHR systems.
• Excellent communication and interpersonal skills.

Responsibilities

• Conduct Comprehensive Needs Assessments (CNAs) and intake documentation.
• Populate client profiles in the EHR with diagnoses, meds, allergies, and other info.
• Coordinate with families, providers, and MCOs to manage referrals and authorizations.
• Respond promptly to emails and voicemails and communicate with care teams.
• Maintain accurate documentation and participate in team meetings and case staffing.
