Job summary

The Fedcap Group in New York, NY seeks a Qualified Health Professional (QHP) to serve as Clinical Assessment Specialist and evaluate a participant’s ability to engage in work activities within the WeCARE program.

You’ll orient clients to the program, review documentation, and interview participants about medical and social factors to determine functional capacity with justification. A master’s degree and 3 years of clinical experience are required.

Qualifications

• Master’s degree in public health, health administration, nursing, social services, or related field.
• 3 years of clinical experience with adults facing medical, mental health, and other barriers.
• Bilingual English/Spanish preferred.

Responsibilities

• Orient WeCARE clients to the WeCARE Program, including all phases of the Clinical Assessment.
• Collect and scan to file all pertinent documentation to support a complete Clinical Assessment.
• Review and discuss prior clinical assessments, case notes, and physician reports.
• Conduct participant interviews covering medical, mental health, housing, employment history, and supports.
• Evaluate existing reasonable accommodations and document changes needed.
• Perform sequential evaluation to determine potential SSI/SSDI eligibility.
• Review all documentation to justify functional capacity outcomes and recommend physician review if needed.
• Explain functional capacity determinations and their impact on WeCARE participation.
• Document services in Fedcap and HRA systems (SEAMS, FedcapCARES).
• Escalate outreach for clients missing appointments.
