Job summary

Equus Workforce Solutions is seeking licensed health professionals to perform comprehensive clinical assessments for disability determinations and care planning. You will review medical records, conduct psychosocial assessments, and determine functional capacity outcomes in coordination with physicians and service teams.

The role requires an active license (LCSW/CRC/LMHC or equivalent), at least four years of clinical assessment experience, and strong customer-service skills.

Qualifications

• Licensed or certified health professional (LCSW/CRC/LMHC or approved equivalent).
• Four years of clinical assessment experience including SSA disability review.
• Experience in fast-paced, performance-driven teams.
• Exceptional customer service and engagement skills; motivational interviewing/care management experience.
• Excellent organization and attention to detail; familiarity with publicly-funded programs.

Responsibilities

• Conduct clinical assessments and reassessments for referred customers.
• Review medical and health documents using standard procedures.
• Perform bio-psychosocial assessments with approved tools.
• Determine functional capacity outcomes and SSA eligibility.
• Coordinate with physicians and treatment teams; ensure follow-up as needed.
• Prepare assessment reports and obtain customer signatures.
• Collaborate with service-track staff to inform care planning.
• Participate in training and adhere to HRA policies and procedures.
