Job summary

Northwell Health's PACE program seeks a Clinical RN Assessment Manager to lead comprehensive assessments for participants, using UAS-NY to determine eligibility and care needs, and to guide personalized service plans.

You will collaborate with intake, enrollment, care delivery teams, and site leadership to ensure accurate documentation, QA, and timely transitions, while adhering to DOH, CMS, and UAS-NY guidelines. Travel across boroughs required.

• Ability to work independently and travel across boroughs

Responsibilities

• Lead Comprehensive Assessments: conduct in-house and virtual assessments using UAS-NY to determine eligibility and care needs, guiding service plans
• Be a PACE Ambassador: articulate PACE benefits to participants, families and referral sources
• Ensure Clinical Precision: review diagnoses/medications and ensure up-to-date documentation in EMR
• Drive Quality & Compliance: adhere to DOH policies, UAS-NY regulations, CMS/PACE guidelines
• Collaborate for Success: work with Intake, Enrollment, Care Delivery, IDT and site leadership
• Advocate for Participants: develop proposed service plans and act as clinical resource post-enrollment
• Innovation & Improve: participate in quality improvement to enhance efficiency and engagement
• Manage Your Impact: schedule 2-3 UAS-NY assessments per day
