Qualifications

• Active NYS Registered Nurse (RN) license.
• PROVEN expertise with UAS-NY.
• FLUENT in Mandarin/Cantonese/Fuzhou Nese/Fujianese Chinese or Spanish.
• Valid NYS Driver's License and a reliable vehicle.

Responsibilities

• Lead with Precision: Oversee the clinical assessment process to ensure tasks are completed efficiently and accurately.
• Master the UAS-NY: Serve as the trusted expert for UAS-NY completion with high compliance.
• Drive Comprehensive Care: Lead comprehensive assessments, onboarding, and reassessments to identify participant needs.
• Guard Participant Safety: Conduct medication reviews and post-hospital discharge evaluations to ensure safe transitions.
• Empower Communication: Use bilingual skills to build rapport and explain care plans to participants and families.
• Optimize Processes: Continuously improve workflows within the clinical assessment function.
• Be a Field Innovator: Perform in-home assessments across Brooklyn.
