Qualifikationen

• Graduate from an ACPE-accredited school of pharmacy.
• PGY1 residency or 2 years of health-system pharmacist experience.
• Texas Pharmacist License or eligible for Texas licensure.
• BLS for Healthcare Providers certification.
• Pediatric Advanced Life Support within 60 days of hire.

Summary:

The Clinical Ambulatory Pharmacist serves as an integral member of the pediatric ambulatory care team, collaborating closely with providers, patients, caregivers, and pharmacy staff to optimize medication therapy outcomes. This role focuses on medication management, patient education, specialty medication access, and care coordination across multiple pediatric specialty clinics.

This position is assigned to multiple pediatric specialty clinics and offers the opportunity to build strong collaborative relationships with providers, patients, caregivers, outpatient pharmacy teams, and specialty pharmacy services to improve medication access, education, and overall patient care.

Key Responsibilities

• Provide comprehensive medication education and counseling to pediatric patients and caregivers.

• Collaborate with multidisciplinary providers to optimize medication therapy and improve patient outcomes.

• Support medication access initiatives, including prior authorizations and appeals, financial assistance programs, and specialty medication coordination.

Education & Experience:

• Graduate from an American Council on Pharmacy Education accredited school of pharmacy PGY1 residency accredited by the American Society for Health-System Pharmacy or 2 years commensurate pharmacist experience in a health-system setting

Certification/Licensure:

• Texas Pharmacist License or eligible for Texas Licensure as a registered pharmacist

• Pharmacist and pharmacy preceptor licensure must be obtained within 90 days of employment

• Current Basic Life Support for Healthcare Providers certification.

• Pediatric Advanced Life Support certification within 60 days of hire (critical care areas only)

Preferred Qualifications:

• Previous ambulatory care pharmacy experience preferred

• Experience working with pediatric patients and specialty medications is highly desirable

• Strong patient counseling, communication, and interdisciplinary collaboration skills
