Job summary

Fairview Health Services in Edina, MN is seeking an In-Clinic Visit Facilitator to support in-person clinic visits within the Primary Care team. This is an 80-hour biweekly, 1.0 FTE position with benefits eligible, with clinic hours Monday-Thursday 7:00 AM–6:00 PM and Friday 7:00 AM–5:00 PM; weekends are not required.

The Visit Facilitator collects pertinent medical information, helps stock exam rooms, and guides patients through the care experience.

Qualifications

• 1 year of customer service experience
• Proficient with technology (computer hardware and programs, smart devices)
• Strong communication and teamwork skills
• Familiarity with healthcare IT systems is a plus

Responsibilities

• Prepare patients for their appointments and collect necessary medical information for the visit.
• Assist in the rooming process by documenting reason for visit, medication review, vitals, and health questionnaires.
• Prepare exam rooms and stock supplies.
• Support the patient connection to the provider and ensure a positive patient/provider experience.
• Assist with scheduling and coordinating follow-up appointments.
• Communicate effectively with patients, providers and other health professionals, in person and via telehealth.
