Job summary

Beth Israel Lahey Health is seeking a Team Nurse RN for an outpatient primary care or family medicine practice in Massachusetts. You will provide direct patient care in a team-based model and support MAs, coordinating care and promoting patient-centered health.

Requirements include RN license in MA and BLS, 1–3 years of experience (new grads considered), CPR, and strong communication. Proficiency with Microsoft Office is expected.

Qualifications

• Graduate from an accredited Nursing Program required.
• License Registered Nurse in Massachusetts required and CPR certification.”

• Strong interpersonal, patient communication and patient education skills.
• Advanced skills with Microsoft applications (Outlook, Word, Excel, PowerPoint or Access).

Responsibilities

• Coordinate and facilitate quality clinical management of a broad outpatient population.
• Collaborates with community providers; follows up on ED visits and discharges; assists with exams and procedures.
• Provide skilled telephonic triage, education and coordination of healthcare resources.
• Perform direct patient care; Nursing visits include AWVs and CCM for BIDHC patients.
