Cynet Health is seeking a Clinic RN to provide evidence-based nursing services to program participants in Grand Junction, CO. The role focuses on teaching, counseling, and skilled interventions to improve health status and ensure coordinated, comprehensive care.

The Clinic RN will support participants and families, perform thorough assessments, develop care plans, and document encounters while maintaining regulatory compliance and prioritizing patient-centered care.

Qualifications

• Excellent interpersonal and communication skills.
• Ability to communicate verbally and in writing in various situations.
• Capacity to interpret a variety of instructions and complete competency evaluations.
• Proficient computer and technology skills.
• Effective time management with ability to focus on multiple priority tasks.

Responsabilités

• Provide evidence-based nursing services to participants enrolled in the program.
• Support participants and caregivers/family members to maintain or improve health status.
• Develop care plans and conduct teaching, counseling, and skilled interventions.
• Coordinate scheduling, documentation, and communication of all care provided.
• Ensure regulatory compliance and deliver comprehensive eldercare.
