Qualifications

• Must lead and direct staff in delivering care.
• Immunizations up-to-date and medically cleared.
• Excellent oral and written communication.
• Ability to standardize processes for quality indicators and use technology to document data.
• Experience with frail elderly population and management preferred.

Responsibilities

• Collaborate with the Physician and Nurse Practitioner to ensure optimal clinical flow.
• Daily workflow management and delegation of tasks to LPN, MA and Scheduling Clerk.
• Triaging and assessment of Participants and providing education resources.
• Supervise LPN and MAs to ensure competency.
• Collaborate with Manager to ensure adherence to quality initiatives.
