Qualifications

• BSN preferred with geriatric care experience.
• Minimum 1 year of work with frail elderly population.
• 5 years of management experience preferred.

Responsabilités

• Collaborates with physicians to ensure optimal clinical flow.
• Directs daily clinic workflow and delegation to LPN, MA, and scheduling clerk.
• Triages participants and provides education resources.
• Supervises staff to ensure competency and adherence to standards.
• Documents clinical activities in participants' medical records daily.
• Coordinates transitions of care with case managers and interdisciplinary team.
• Works with Nurse Educators on staff education and competency.
