Job summary

PACE Southeast Michigan is seeking a Clinic Registered Nurse Coordinator to ensure optimal clinical flow and quality care. The role requires supervising LPNs, MAs, and scheduling clerks, and collaborating with physicians and nurse practitioners.

The RN Coordinator will use nursing processes to assess, plan, implement and evaluate care, with emphasis on frail elderly participants and family involvement in care outcomes.

Qualifications

• BSN preferred.
• 5+ years of management experience preferred.
• Ability to lead and direct staff in delivery of care.
• Excellent oral and written communication skills.
• Ability to document and present data using technology.

Responsibilities

• Collaborate with Physician and Nurse Practitioner to ensure optimal clinical flow.
• Daily workflow of clinical activities and delegation of tasks to LPN, MA and Scheduling Clerk.
• Triaging and assessing Participants in the clinic and providing education resources.
• Supervise LPN and MAs to ensure competency of clinical skills.
• Ensure documentation of clinical activities in the Participants medical record daily.
• Coordinate with RN Case Managers on care and transitions of care.
