Job summary

Responsibilities

• Interview for nursing history, assess health knowledge and maintenance, and document findings in the medical record.
• Obtain medical information, chief complaints, vital signs and test results for physician examination and record documentation.
• Advise patients and families; address concerns about service and care quality.
• Evaluate need for referrals to Home Health or community agencies.
• Screen patient problems via in-person or phone with advice/referral to physician.
• Provide education on prevention, health maintenance, disease information; document results.
