Job summary

Mille Lacs Health System seeks a Clinic RN to collaborate with patients, families and care teams in delivering quality, evidence-based care in the clinic setting. Responsibilities include patient rooming, injections, medication reconciliation, care coordination with providers, and ongoing patient education.

Strong communication, organization, and EHR proficiency are essential in this role. Mon–Fri daytime hours with occasional evenings; commitment to exceptional patient experience and community

Qualifications

• Graduate from an accredited nursing program with Minnesota RN licensure.
• Excellent communication skills across disciplines and at various levels.
• Attention to detail, organization and prioritization skills.
• Basic computer skills and BLS certification.

Responsibilities

• Perform nurse visits and facilitate quality initiative workflows.
• Assist staff to enable efficient patient care flow; rooming, injections, and coordination of outside referrals.
• Provide medication reconciliation and continuity of care with pharmacy and other providers.
• Facilitate medication refills by protocol.
• Connect patients to community resources and services.
• Coordinate anticoagulant program development and care plan adherence.
• Educate patients and coordinate care with primary providers and families.
• Reduce ER visits and readmissions through chronic care coordination.
