Job Title: Clinic RN
Position Type: Full-Time
Department: Nursing

Job Summary

Has the responsibility for providing direct and indirect nursing care to assigned participants; Works under the direct and indirect supervision of the Clinic Supervisor or designee but is independent in the application of advance nursing knowledge and skills; Manages complex clinical situations for which he or she is responsible.

Specifications

Education and Special Training: Graduate of an accredited school of nursing.
Experience: Has practiced nursing in the last 3 years; 3 years RN experience with at least 24 months experience working with the aging, frail or elderly populations.
Licensure, Registry or Certification Required: RN currently licensed in North Carolina (NC); presents credentials which demonstrate ability to meet performance criteria for RN; Valid NC Driver’s license and vehicle.
Knowledge, Skills, and Abilities: Must possess knowledge and skills necessary to treat participants; Only act within the scope of his or her authority to practice; Meet a standardized set of competencies for the specific position description established by PACE of Guilford and Rockingham Counties and approved by Centers for Medicare and Medicaid Services (CMS) before working independently.

Key Responsibilities

  • Assessment: Collects participant data and completes required documentation; identifies participant’s overt problem/needs and sets priorities; identifies problems requiring further referral and/or follow-up; interprets and records latest diagnostic results; performs advanced nursing assessment using critical thinking skills.
  • Planning Care: Develops a plan of care based on nursing process which incorporates the plans of other disciplines and continuing care needs; includes the participant/family in developing or revising the plan of care; makes referrals to interdisciplinary support services. Initial and periodic assessments (minimally every 6 months) to be completed prior to the scheduled team meeting. Communicates participant changes to team members.
  • Implementing/Managing Care: Care provided conforms to accepted practice standards; provides correct treatment/procedures and other care as prescribed and according to patient care standards; demonstrates understanding of age-related characteristics and needs of participants served; explains nursing procedures in appropriate ways; provides participant and family education incoordination of 24 hours care delivery; identifies emergency situation and independently initiates advance therapy; acts a coordinator of patient care with other health care personnel and evaluates patient care measures instituted; understands and demonstrates respect for participant rights and utilizes established mechanisms for management of ethical issues in patient care.
  • Evaluation: Records participant care delivered as planned and any variation, with appropriate rationale; makes and records observations related to impending or associated problems; implements nursing measures related to impending or associated problems.
  • Leadership: Using nursing process, delegates nursing care to appropriate personnel; integrates cost effective measures into nursing practice; recognizes unite problems and takes responsibility for documenting same; demonstrates active participation in the Quality Assessment/Performance Improvement (QAPI) process; complies with expectations to meeting staffing demands based on participant care needs; complies with policies addressing safe working conditions; monitors unsafe working conditions; recognizes inappropriate and/or ineffective participant care management; resolves issues/problems and completes written reports; fulfills care of unit assigned and adjusts staffing for census (volume) and acuity.
  • Participates as a member of the Interdisciplinary Team (IDT) and in participant care planning.
  • Fulfills duties of the Health Care Coordinator as outlined in the NC Adult Day Health Certification Guidelines: completing preadmission health assessment, implementing the health care components of the established service plan, monitoring participant’s response to medical treatment plan and nursing interventions, revision of the plan of care as necessary, reporting and recording results of the nursing assessment, care rendered and participant’s response to care, collaborating with other health care professionals and caregivers regarding provision of participants’ health care, educating other staff members to emergency procedures and providing information to staff and caregivers about health concerns and conditions of participants, providing first aid treatment as needed, making sure health personal care services are provided to participants and consistent with the service plan.
  • On-call rotation as assigned by Clinical Coordinator.
