The ideal candidate has an RN license, a nursing degree, and at least two years in clinic leadership for a frail elderly population, plus CPR/First Aid certification within 180 days of hire.

Qualifications

• Bachelor’s degree in nursing or relevant healthcare field.
• Unencumbered RN license required.
• Minimum of two years overseeing ambulatory clinic operation.
• Experience with frail/elderly population.
• CPR and First Aid certification within 180 days of hire.

Responsibilities

• Operate a high-performing, well-equipped clinic with efficient workflows and clear protocols.
• Hire, retain, coach, develop and supervise RN Case Managers, Clinic RNs, Medical Assistants, and other staff.
• Analyze data for trends and opportunities; perform root cause analyses to improve care delivery and operations.
• Ensure staff document interventions per care plans and promptly respond to physician orders.
• Lead CMS/DHCS audit activities with leadership and develop corrective action plans.
