Job summary

Open Arms Solutions is seeking a relationship-driven Client Transition Coordinator to be the face of our organization with skilled nursing facilities, rehabs, and senior living communities across the near western and western suburb area. You will connect families with the care they need exactly when they need it.

You'll build partnerships, educate staff on our private duty senior care services, and coordinate smooth handoffs with our intake and care teams.

Qualifications

• 2+ years in social work, home care, home health, hospice, or senior living business development or marketing.
• Natural relationship-builder comfortable walking into facilities and starting conversations.
• Strong communication and presentation skills.
• Familiarity with private pay, Medicaid waiver, or long-term care insurance funding.
• Reliable transportation and a valid driver's license; field-based role.
• High school diploma required; Bachelor's degree preferred.

Responsibilities

• Build relationships with social workers, discharge planners, directors of nursing, and resident services directors at SNFs, rehabs, and CCRCs.
• Visit assigned facilities regularly to educate staff on private duty senior care services.
• Turn referrals into new clients by coordinating smooth handoffs with intake and care team.
• Develop, host and attend community events, health fairs, and networking functions to expand referral base.
• Track outreach and results, and hit monthly referral and admission goals.

• Turn referrals into new clients by coordinating smooth handoffs with our intake and care team

What We're Looking For

• 2+ years in social work, home care, home health, hospice, or senior living business development or marketing (preferred)
