With over 10 years of experience and a strong national presence, our turnkey chronic care coordination programs provide simple access to care coordination from anywhere, helping health plans deliver better outcomes for their members.

We are seeking a dynamic and adaptable Client Success Relationship Manager to join our Client Services team. This role involves client-facing interactions, identifying and delivering sales and growth opportunities, supporting client implementations, nurturing relationships, presenting program value, driving account growth, and ensuring client success. The position is fully remote but requires residence in California.

Key Responsibilities
  • Establish and maintain strong relationships with clients through regular outreach and communication.
  • Serve as the main point of contact for clients, understanding their needs and addressing concerns promptly.
  • Deliver impactful presentations demonstrating the value of our products and services.
  • Track and manage contractual commitments, ensuring timely delivery of services.
  • Partner with internal departments such as clinical services, operations, and analytics to meet client expectations.
  • Facilitate effective communication between clients and internal teams, conveying client feedback and requirements.
  • Identify opportunities for upselling or cross-selling additional services based on client needs and market trends.
  • Engage directly with community members to understand their needs, concerns, and preferences.
  • Leverage community feedback to inform strategic decisions and initiatives.
  • Maintain a strong community presence, demonstrating commitment to making a positive impact and supporting well-being.
  • Travel up to 25% to client sites for in-person meetings.
Required Qualifications
  • BA/BS Degree or equivalent
  • 4+ years in a client-facing role
  • Resides in California
  • Fluency in English and Spanish, verbal and written
