Qualifications

Responsibilities

• Implement and evaluate individualized programs for clients, including treatment team plan and safety plan.
• Assist with client ID processes (Food Assistance, SS Cards, State ID, Birth Certificate).
• Manage development and implementation of Individual Program Plans (IFP/POC data changes).
• Participate in treatment team meetings coordinating between agencies and clients.
