Job summary

IM Houston is seeking an Intake Specialist to answer and handle all client intake calls, complete intake assessments, determine eligibility for Meals on Wheels programs, and coordinate referrals.

You will work with assessors, maintain waiting lists in the Caseworthy ServTracker, and ensure accurate client documentation while supporting multilingual communication and efficient service delivery.

Qualifications

• High school diploma or equivalent required.
• Associate degree or higher preferred.
• Minimum two years of customer service and clerical experience.
• Bilingual in English and Spanish.
• Proficiency with Microsoft Office and databases.
• Strong organizational and interpersonal skills.
• Detail oriented with good communication and reliability.

Responsibilities

• Complete intake assessment on all prospective calls for Meals on Wheels.
• Secure information from callers about medical, psychological, and social factors and direct inquiries to staff.
• Provide referral information to callers needing assistance from other organizations (including United Way).
• Refer eligible clients to IM programs and services (such as Animeals).
• Maintain contact with assessors and relay client information when needed.
• Maintain client services data and waiting lists.
• Fill out Change of Status forms when needed.
• Enter inquiries into and maintain MOW waiting list in Caseworthy ServTracker.
• Determine client’s initial eligibility for IM services.
• Code intakes to the appropriate route.
• Assist in developing flyers for MOW clients.
• Compile records and prepare reports from SERVtracker.
• Act as a backup to the Client Services Coordinator.
• Document and address client concerns.
• Other duties as requested by supervisor.

Position Summary

Answer and handle all client intake calls, complete intake assessments, determine baseline of client eligibility, direct client inquiries, assist in coordinating assessors and provide referrals.

Main Responsibilities

• Complete intake assessment on all prospective client calls for Meals on Wheels
• Secure information from callers such as medical, psychological, and social factors contributing to client’s situation and direct inquiries to client services staff.
• Provide referral information to callers needing assistance from other organizations (including United Way). Refer eligible clients to other IM programs and services (such as Animeals).
• Maintain contact with assessors. Assist in relaying client information and needs to assessors when needed.
• Maintain client services data (number of re-/assessments completed, attrition, waiting list, etc.)
• Fill out Change of Status Forms when needed.
• Enter inquiries into and maintain MOW waiting list in client management system, Caseworthy ServTracker.
• Determine client’s initial eligibility for IM services using discretion and independent judgment with respect to the MOW qualifications and explained client situation.
• Code intakes to the appropriate route.
• Assist in developing flyers for MOW clients.
• Compile records and prepare reports as needed from the SERVtracker system.
• Act as a backup to the Client Services Coordinator.
• Document and address client concerns.
• Other duties as requested by supervisor.

Qualifications, Skills & Experience

• High school diploma or equivalent is required. Associate degree or higher is preferred.
• Minimum two years of customer service and clerical experience.
• Bilingual (English/Spanish).
• Computer proficiency required. Experience using Microsoft Office (esp. Excel) and databases.
• Must be dependable and have good organizational and interpersonal skills.
• Strong and proven attention to detail, interpersonal and computational skills.
• Detail oriented with good organizational skills and effective verbal and written communication skills.
• Must have reliable transportation, valid driver’s license, and automobile liability insurance.
