Qualifications include a Bachelor’s degree in a health-related field; licensure as a social worker, RN, or counselor is preferred. Candidates must be legally authorized to work in the United States.

Formación

• Bachelor's degree in a health-related field from an accredited four-year college or university.
• Licensed as a social worker (any level), RN, or counselor preferred. License must be maintained.

Responsabilidades

• Assess clients' needs and goals, and develop treatment plans with clients.
• Gather data, obtain releases, and coordinate records relevant to continuity of care.
• Refer and link clients to community services and resources aligned with the plan; monitor outcomes.
