The State of Nevada, Aging and Disability Services Division, is recruiting for a Health Care Coordinator 1 in Las Vegas. Position serves to underfill and may advance after performance and endorsement by the appointing authority.

Responsibilities include case management, assessments, and documentation using a web-based system to support HCBS and FOCIS/MFP programs. Under supervision, this entry-level role performs medical service evaluations, eligibility determinations, and coordination of care,

Qualificações

• Licensure as a Social Worker or professional licensure in a medical specialty applicable to the assignment.
• Experience in case management and Medicaid services.
• Ability to assess, plan, implement and oversee care for clients.

Responsabilidades

• Assess potential client needs for case management and develop care plans.
• Conduct home visits and coordinate with providers and services.
• Review and document billing and Medicaid eligibility and services.
• Maintain client records and ensure compliance with policy and procedures.
