Qualifications

• Bachelor's degree in administration, social work, public health, or related field required.
• Minimum of two years' experience in health care or human services; substitution for a related degree may apply.
• Knowledge of Medicaid and Medicare funding sources.
• Experience in conducting comprehensive, person-centered assessments in community settings.
• Strong advocacy and communication skills with self-determination as a key component.
• Ability to build rapport with individuals, families, and community resources.

Responsibilities

• Conducts comprehensive, person-centered assessments in the person's preferred setting with involvement from family or representatives as requested.
• Educates individuals on program components, service options, DSS guidelines, eligibility, costs, and pros/cons of each option.
• Promptly completes all client documentation, applications, forms, and related paperwork.
• Monitors and reviews care plans, service delivery, and contract obligations; revises plans with clients as needed.
• Conducts person-centered telephone and in-person interviews for reassessment and monitoring.
• Works with an interdisciplinary team and participates in on-call coverage and other resources.
• May participate in mentoring new staff and continuing education services.
