Wheeler Clinic seeks a Client Care Coordinator in New Britain, CT to support client engagement and act as liaison with DOC/CSSD referral sources. The role requires timely correspondence, data entry, and assisting uninsured clients with Medicaid applications as needed.

Qualificações

• High School diploma required with 2 years of progressively responsible work experience.
• Bachelor's degree preferred.
• Demonstrates attention to detail and professional communication.

Responsabilidades

• Liaises between the program and the DOC or CSSD referral source and acts as point of contact.
• Coordinates with staff, referral sources and clients to support engagement and retention.
• Provides outreach to hard-to-engage clients and facilitates orientation as needed.
• Maintains timely and accurate documentation, reports and data entry in required systems.
• Ensures timely communications with referral sources and prepares required reports and releases.
