The role develops an implementation strategy based on participants' Personal Plans, aligns with the agency's vision, and embraces person-centered, collaborative approaches.

Qualifications

• Bachelor's degree in human services field and one year experience with ID/DD.
• Ability to read, write, and communicate effectively.
• Willingness to engage in person-centered philosophy and collaborative approach.
• Must have valid driver's license and provide proof of insurance.

Responsibilities

• Participate as a full member of the participant's support network; outline and implement Personal Plan; adjust as needed.
• Complete required documentation timely to ensure compliance.
• Facilitate community engagement and develop relationships for employment, volunteering, and personal interests.
• Report participant progress to the participant and support network.
• Establish constructive working relationships with participants and their network.
• Monitor money management and entitlements; ensure compliance with Social Security and Illinois programs.
• Communicate agency plan and philosophy to participants and staff.
• Coordinate Direct Support Personnel training per DHS curriculum; maintain documentation.
• Promote volunteerism and public awareness to position Community Link as a leader.
