Job summary

Qualifications

• Excellent communication and interpersonal skills.
• Bilingual English/Spanish a plus.
• Current Basic Life Support (BLS) required.
• Current license to practice professional nursing in Texas required.
• ACLS certification preferred.

Responsibilities

• Assess barriers when patient has not met treatment goals or missed appointments.
• Assist patients in setting SMART goals for self-management and report progress.
• Collaborate with payer Case Managers for additional services when appropriate.
• Collaborate with physicians and care team to identify and manage suitable patients for care management.
• Document patient self-management measures and progress toward goals.
