Qualifications

• Excellent communication and interpersonal skills. Good oral, written and presentation skills.
• Current license to practice professional nursing in the state of Texas required.

Responsibilities

• Assess barriers when patient has not met treatment goals or appointments.
• Assist patients in setting SMART goals for self-management.
• Collaborate with payer Case Managers for additional services.

Education

Bachelor of Science in Nursing

Current license to practice professional nursing in Texas

Advanced Cardio Life Support Certification
