Responsibilities

• Provide ongoing support to patients with chronic conditions through outreach, care coordination, education and disease management.
• Perform medication reconciliation and assess adherence during patient encounters.
• Assist with enrollment into Chronic Care Management and Remote Patient Monitoring programs.
• Maintain HIPAA compliance and accurate documentation while following policies and regulatory requirements.
