Job summary

Ozarks Community Hospital in Springfield, Missouri seeks a Chronic Care Management Specialist to join the Medical Home department. This role includes traveling with the Long Term Care Team to nursing facilities in the area.

Responsibilities include direct/indirect patient care, CCM enrollment and follow-up, and coordinating documentation and billing under supervision of nursing/medical leadership.

Qualifications

• High school diploma or equivalent.
• Successful completion of a CNA, MA, or LPN/LVN program.
• Current BLS certification at hire or during orientation.
• Valid Driver's License with background check during onboarding.

Responsibilities

• Provides direct and indirect patient care in office/clinic settings.
• Oversees Chronic Care Management enrollment, follow-up, and documentation.
• Manages reporting and billing procedures related to CCM.
• Coordinates with providers and Long Term Care Team during travel to nursing facilities.
