What You’ll Do
As a Patient Care Coordinator, you’ll play a vital role in our patients’ health journeys by conducting monthly telephonic outreach, assessing their needs, and creating individualized care plans. Your responsibilities include managing patient care through scheduled calls, documenting visits using technology platforms and EHRs, developing care plans focused on physical, mental, and preventative health, coaching patients on treatment plans, wellness, nutrition, and goal setting, and helping patients prepare for medical appointments and connect with resources.

What We’re Looking For
Active Compact LPN license, at least 2 years of clinical experience (care coordination preferred), confidence in using EMRs and digital tools, and strong communication and problem-solving skills.

Schedule
Full-time, 40 hours/week, Monday–Friday between 8 AM – 6 PM PST/MST. Referral Bonuses: Up to $1,000.

What To Expect After You Apply
Our hiring process typically takes 2–3 weeks and includes a timed assessment to evaluate computer literacy, basic math, and clinical knowledge. Strong tech skills are essential as this role is fully remote. Candidates who pass the assessment will have virtual interviews with our Recruiting Manager and Clinical Director.
