Optima Medical is seeking a Care Coordinator for its CCM program. This in-office, phone and EMR-based role supports patients with chronic conditions, helping them stay on track with care plans and health goals. Medical Assistants with hands-on clinical knowledge are encouraged to apply.

The position emphasizes coordination with healthcare providers, patient education, and use of EHR tools to ensure continuity of care in a dynamic team environment.

Qualificações

• One year of clinical work experience as a Medical Assistant.
• EMR experience, with eClinicalWorks preferred.
• High level of professionalism required.
• Excellent written and verbal communication skills.
• Strong customer service skills.

Responsabilidades

• Develop personalized care plans with patients, caregivers, and providers.
• Coordinate and facilitate communication among healthcare team members.
• Monitor adherence to treatment plans and medications.
• Provide ongoing education and support on disease management and prevention.
• Utilize EHR and tools to document encounters and track interventions.
• Collaborate with community resources to address social determinants of health.
• Conduct regular follow-ups to optimize health outcomes.
