Job summary

Optima Medical in Arizona is seeking a Care Coordinator to join our CCM program. This in-office, phone and EMR-based role supports patients with chronic conditions by coordinating care, outreach, and follow-up to achieve health goals.

Ideal candidates have Medical Assistant training, EMR experience (eClinicalWorks preferred), excellent communication, and a patient‑centered approach. Bilingual English/Spanish is a plus.

Qualifications

• Medical Assistant training and CMA/MA diploma preferred.
• EMR experience, eClinicalWorks preferred.
• Strong communication and customer service skills.
• Professionalism and reliability in office settings.

Responsibilities

• Develop personalized care plans with patients and caregivers.
• Coordinate communication among physicians, nurses, and care team.
• Monitor adherence to treatment plans and follow-up on progress.
• Educate patients and families on disease management and preventive care.
