Job summary

The Guidance Center in Michigan seeks a dedicated Case Manager to assess and coordinate services for children and families. You will link clients to community resources, develop Person-Centered Treatment Plans, and provide in-home and community-based support, with regular follow-up to monitor progress.

We offer a $3,000 sign-on bonus and a Monday–Friday schedule 8:30–5:00, with after-hours and weekend coverage as needed, supporting a team that fosters resilience and well-being.

Qualifications

• Bachelor degree in social work, psychology, or related field.
• Must obtain Children’s Mental Health Professional credential within 120 days of hire.
• 3 years experience with minors and families or master’s plus 1 year experience.
• Valid driver’s license or must obtain one within 30 days of employment.

Responsibilities

• Conduct case management assessments for children and families.
• Gather information from clients, families, guardians, and reports across agencies.
• Develop comprehensive Person-Centered Treatment Plans with the treatment team.
• Provide face-to-face interventions with clients in office and community settings.
• Maintain monthly contact with treatment team to coordinate services.
