Job summary

Community Healing Centers in Kalamazoo, MI, seeks a Social Worker (LMSW) to join the Outpatient Adult Substance Abuse/Mental Health Services team. The role supports clients through assessments, treatment planning, and individual, group, or family therapy within integrated care.

The position is full-time with potential contractual terms. You will perform admissions, ongoing therapy, and coordinate with funding authorities to ensure access to services; some evening groups may be required.

Qualifications

• Master’s Social Worker Clinical License required.
• Experience providing Substance Abuse counseling to individuals and groups.
• Knowledge of CMH population and required documentation.

Responsibilities

• Provide outpatient clinical services including psycho-social assessments, treatment planning, and discharge/aftercare planning.
• Conduct individual, group and/or family sessions.
• Perform admission, assessment, and discharge functions.
• Assess mental health, substance use, and other status of clients.
• Prepare treatment plans and submit reports per CARF and state requirements.
• Obtain funding authorizations from the state and insurers.
• Maintain professional relationships with referral sources.
• Note: this position may run some evening groups.
