Job summary

A&I Avenues in Colorado's front range seeks a Children’s Case Manager to provide case management for clients aged 0–21, conduct eligibility determinations, develop and implement service plans, and coordinate care with clients, families, guardians, and providers.

The role requires strong communication, independent judgment, and the ability to document in accordance with federal and state regulations. Field days are required, and a personal vehicle and mobile device are used regularly.

Qualifications

• Bachelor’s in human services or related field preferred.
• Experience with disability services and case management.
• Ability to document accurately to meet funding requirements.

Responsibilities

• Complete face-to-face assessments to determine eligibility for long-term care.
• Lead annual review and development of service plans for clients.
• Ensure service plan documentation is completed and distributed.
• Coordinate delivery of services and supports with families and providers.
• Educate clients and families on service options and assist in obtaining needed supports.
• Monitor services quarterly and ensure alignment with goals.
• Complete enrollments, terminations, and appropriate notices.
• Provide crisis resources and connect clients to needed community supports.
